Universal Medical Form
(UMF)

This page contains information about the UMF -- an essential part of the request for accommodation packet.

About The UMF:
In order to strengthen our cause, the Coalition has created our own Universal Medical Form (UMF). We would like all complainants to include a completed copy of this form when they mail off their request for accommodation letters.

This form consists of the following:

Page #1 -- To be filled out by the individual. On this page, the individual will use his or her own words to describe their disability and how this impacts their ability to participate in the archery season. The individual will sign this page.

Page #2 -- To be filled out by a doctor. On this page, the doctor will use his or her own words to describe the individual's disability, and how this impacts the ability to participate in the archery season. The doctor will sign this page.

Both pages will become part of the formal request for accommodation packet.

Because this form is "universal", it can be used for multiple states. In other words, a complainant should only have to visit the doctor one time have this filled out, and then photocopies can be made for the multitude of states.

The purpose of the form is to strengthen our cause, and reduce the likelihood that the state can take delay tactics, or sneak out of their responsibilities. The Coalition is concerned that states, in an effort to debunk our cause, may ignore the crossbow issue, and try to focus all of their efforts on disproving our medical conditions. In other words, if the state can prove we aren't disabled, then they can immediately dismiss our disability discrimination complaints. Having this form filled out completely will force the states to recognize the fact we are truly disabled. This will keep the focus on the state, and whether their practices are discriminatory.

In addition, this form will help show unity and determination. The states will know that we mean business simply because we took the time to organize, develop our own forms, and then have these forms signed by a doctor.

Finally, the Coalition is trying to ensure that our members do not have to make multiple trips to the doctors office. Having this form filled out completely will reduce the likelihood that states will demand more details from us. This form may also be enough to appease the Department of Justice -- meaning that our members shouldn't have to re-visit the doctors if ADA complaints are filed.

Miscellaneous Instructions:

We encourage all members to fill out Page #1 using a word processor on their computer.

Page #1 is designed to be filled out on your computer. (using a word processor such as Word, Works, or Word Perfect.) Simply click on the boxes under each question and start typing. You can use your tab key to move inside a box. When you're finished, print off both pages and visit your doctor. We encourage all members to use your computer for Page #1 to ensure your answers are legible.

Please have your doctor to answer Page #2 completely and honestly. Let the doctor know that this form is very important for your case. The doctor needs to take his or her time and answer all questions as thoroughly as possible. The more your doctor writes, the stronger your case. If the doctor says you aren't disabled enough for a crossbow, then you shouldn't be mailing a complaint. We'd rather have your personal doctor shoot down your case, then a doctor representing a state.

Please have your doctor to write legibly. Doctors have a nasty habit of writing scribble on paper. The doctor's writing will be useless if it can't be read by the state.
